Pharmacists can often identify the people we are caring for who live with diabetes. But how many of us know which of the one in three people with diabetes that we are caring for who also have diabetic kidney disease? This session will share how pharmacists can actively prevent and manage the chronic condition of diabetic kidney disease through:

• Timely, wisely-chosen, screening
• Use of evidence-informed cardiorenal protective therapies where indicated, including the established GLP-1 RA agent and its recent demonstrated benefit in the management of DKD
• Documentation to support interprofessional collaboration and care of the individual living with diabetic kidney disease
