Infection does not just make patients with chronic disease sicker. It destabilizes the disease itself. Influenza, COVID-19, RSV, pneumococcal disease, and herpes zoster all trigger myocardial infarction, stroke, and exacerbations of COPD and heart failure, and the elevated risk persists long after the acute illness resolves. Pharmacists see these patients constantly, which makes every chronic disease interaction a chance to prevent a downstream health event. This session reframes vaccination as core chronic disease management: recognizing which adults carry the greatest risk across all five vaccine-preventable diseases, connecting that risk to eligibility under current NACI guidance, and making a recommendation patients accept. Participants leave with a one-page point-of-care tool.
