The community pharmacy is the destination for Canadian adults to fill their regular medications and receive annual influenza vaccinations.  The current primary care crisis across the country has started the mobilization of pharmacists in chronic disease management (CDM).  This session is more than just Influenza.  Learn how influenza vaccination is a powerful intervention within a chronic disease management strategy to support patients diagnosed with COPD, CKD, DM, and Heart Failure.   Engaging cases reflecting clinical guidelines for these common chronic diseases will be discussed.  Attendees will gain insights on how regular influenza vaccination and scheduled CDM visits can be applied to the community pharmacy setting.
