With advancements in cardiorenal and non-hypoglycaemic affiliated pharmacotherapies for type 2 diabetes, the possibility exists to significantly reduce hospitalizations and complications of type 2 diabetes. In 2020, the Diabetes Canada Clinical Practice Guidelines updated the Pharmacologic Glycemic Management of Type 2 Diabetes chapter establishing the priority of cardiovascular and renal protective medications in high-risk populations. However, three years later, many people affected with type 2 diabetes, remain on sub-optimal management.

This engaging session will demonstrate how using the pharmacy workflow environment, and a step-wise approach, pharmacists can improve health outcomes in our communities. Participants will be able to identify individuals who could benefit from our help, and then, using resources available on websites, conversations in a community pharmacy can be supported.
