When access to primary care is delayed or unavailable, every missed opportunity for early detection can become a preventable crisis. This presentation explores how point-of-care testing can be successfully implemented in a  community pharmacy to close critical gaps in care. It examines practical strategies for integrating POCT into everyday pharmacy workflows, including team engagement, training, patient identification, and collaboration within the circle of care.
